Event Attendance Waiver and Consent Form

Purpose:

This form is intended to confirm that by attending Knoxville Gay Social events, whether as a non-member or as an underage participant, you have read, understand, and agree to the full Terms of Service and the conditions outlined below.

Information entered into this form will be handled in accordance with our Privacy and Data Usage policy found in the Terms of Service.

Acknowledgment of Risk and Release of Liability

• Knoxville Gay Social, its leadership, and associated volunteers are not liable for injuries, damages, or losses arising from your participation.

• You agree to abide by all event rules and leadership instructions during your participation.

I further understand that I must receive approval from the Leadership Team and/or Subgroup Moderator prior to attending any event.
